The type of personal information we collect

We currently collect and process the following information:

  • Personal identifiers, contacts and characteristics (for example, name and contact details, reason for contact) on our initial ‘contact us’ form. This is so that we can contact you regarding an appointment.

  • Either prior to or at your first appointment, we will ask you to complete a consultation form which includes data regarding:

    • your name, address, DOB, telephone number,

    • your relevant medical history,

    • Information on current state (e.g. pains, aches etc)

  • After each appointment I will record your treatment details and assessment notes.

How we get the personal information and why we have it

Most of the personal information we process is provided to us directly by you to enable us to provide you with safe and effective therapy appropriate to your needs. It may also be used to refer to at subsequent treatments in order to assess levels of improvement. Your information will not be shared with anyone (other than for legal process) without explaining the reason why this is necessary and obtaining your explicit consent.

Your contact information may be used by BRSTT to contact you in relation to appointment times or dialogue regarding your treatment plan.

Under the UK General Data Protection Regulation (UK GDPR), the lawful base we rely on for processing this information is: Legitimate interest. This means that BRSTT will process and retain the information so that you can be provided with the best possible treatments options and advice.

How we store your personal information

Your information is securely stored. BRSTT collects information both online and on paper. Paper information forms and treatment notes are securely locked in a filing cabinet. Information collected electronically is held on a password protected computer in an encrypted folder.

BRSTT is required to keep your information for a period of 7 years after your last treatment for insurance purposes. Your data will not be transferred without your consent. After this time your records will be destroyed.

Additional notes

If an individual does not consent to BRSTT keeping records on information about an individual and treatment records then it may not be possible to provide the therapy.

BRSTT has to keep records of your treatment for a certain period as described above which means that even if you ask them to erase your data, they might need to keep these details until that time period has elapsed.
